Stages of pressure injures are:-

  • stage 1- mildest and affects only upper layer of skin, wound is not open yet. Appears red, warmer temperature and sore to touch
  • stage 2- sore area of skin has broken through the top layer(epidermis) and some layers below(dermis). Appears as a shallow crater -like wound with serous filled blister.
  • stage 3- sore has been progressed and broken completely through the top two layers of skin and into fatty tissue below, resembles hole and fat is visible, with foul smell and pus discharge.
  • Stage 4- most serious and sore extends below the subcutaneous fat into deep tissue(muscles,tendon), with high risk of infection. There is extreme pain, dead tissue appears black, pus and foul smell.
